Our Lady of the Assumption Ladies Guild 2010 Service Award

 

Each year, the Ladies Guild of Our Lady of the Assumption grants service awards to graduating high school seniors who are members of the parish and have shown a regular and continual commitment of service to Our Lady of the Assumption. In order to be eligible, applicants must:

  • Be a member in good standing at Our Lady of the AssumptionParish (weekly attendance at Mass and reception of the sacraments)

  • Be a high school senior from any area high school

  • Have a minimum of 3 years active service since Confirmation to Our Lady of the Assumption Parish in one of the following areas: altar service, choir, usher, lector and/or religious education teacher

  • Complete the application packet which is available to download here or at the Office of Religious Education in the Church Rectory (office hours are Mon.-Thurs. 10-5pm). 

  • Complete application packet must include: 

    • Candidate application form

    • Essay

    • OLOA recommendation

    • School/Employer/Community recommendation

    • High School picture
